CASE MANAGEMENT
BEHIV’s Case Management Team is available to assess your HIV-related needs, to provide information about available resources, and to develop a service plan designed to meet your needs.

Case Managers are knowledgeable about the resources available to people living with HIV and AIDS, including medical providers, treatment programs, entitlement programs (i.e. public aid and Social Security), housing, and mental health services, among others.

BEHIV participates in the State of Illinois, Department of Rehabilitation Services Case Management program which assists persons disabled due to HIV/AIDS to receive homemaker services.

Bilingual services are available for Spanish speaking clients.

For more information about Case Management, call 773.293.4740 or
e-mail casemanagement@behiv.org
